Dose of steroids in gout

I have never written a review on anything really, but I thought it was important in this case. I have been dealing with arthritis for probably 15 years. I was essentially given two choices: the first is take Allopurinol and hope it prevents me from getting sprouts. However, I would have to take a 20 mg pill every day, and this medication has upset my stomach in the past. Option two is to treat an outbreak when it occurs with the medication. I went this route, and took Colchicine, which is the medication to treat this. The problem is that it never seemed to work, and I recently had a terrible gout attack. I got Prednisone and it really worked miracles. I was probably 70% better after one full day, and now almost 100% after three days. I am more than satisfied with this medication!

The third category of medications are those used during attacks of acute gout to decrease pain and inflammation. Both colchicine (Colcrys) and NSAIDs can be used during an acute gout attack to decrease inflammation and pain. Corticosteroids such as prednisone , methylprednisolone ( Medrol ), and prednisolone ( Orapred ), also can be used during an acute gouty flare. However, the total dose of steroids is generally limited due to potential side effects such as cataract formation and bone loss. Steroid medications are extremely helpful in treating gout flares in patients who are unable to take colchicine or NSAIDs.

"May 2016, white male 51 yrs old, first gout attack. I was lifting heavy about 2 weeks ago and I over did red meat, chicken, salmon, two protein shakes per day at 30 grams each, aspirin almost daily and a 5 hour energy drink, junk food in there for good measure. Drink instant coffee excessively daily Did not like to drink water so likely became hydrated as well. I feel I brought this on with my eating habits but blood test came back uric acid which was within normal range. I in total pain and missed 2 weeks of work. Doctor put me on Indomethacin after weeks of Advil, water, cherries, cherry juice, no red meat or alcohol, etc with no relief. Felt relief first hour. Did get a little dizzy but pain is almost gone after 1st pill."

If a gout attack occurs while you are taking allopurinol, you can still take an anti-inflammatory painkiller to relieve the pain. However, this may indicate that you need an increased dose of allopurinol. Side-effects are uncommon with allopurinol. Read the information that comes with the packet of tablets for details about possible side-effects. If side-effects do occur, other medicines with a similar action are sometimes prescribed. For example, a medicine called febuxostat may be an option if you cannot take allopurinol for medical reasons or due to side-effects.

4) monthly uric acid levels may be excessive and are unlikely to change management; UpToDate suggests “Serum urate levels should be monitored to assure maintenance of concentrations in the goal range and to permit antihyperuricemic drug dose adjustment as needed. One approach is to determine serum urate concentration within two to four weeks of a dose adjustment, with confirmation three months later. Once goal values are confirmed, measurement every six months for the next year and then annually is usually adequate, unless drugs or lifestyle factors potentially altering urate levels have been introduced in the interim.”
